Extraction Orthodontic Treatment of Maxillary Central Incisors with External Root Resorption and Ankylosis due to Trauma

Abstract
Even though extraction of maxillary central incisors is an unusual decision, it could be a reasonable choice in the case of severe root resorption of incisors. To close the extraction space and substitute the ipsilateral lateral incisors to central incisors, careful diagnosis and orthodontic treatment planning must be taken to ensure the anterior dental esthetics. A 14-year-old woman patient visited with complaints of protrusion. The right maxillary central incisor showed severe external root resorption and the left one showed ankylosis resulting in a significant difference of the incisal edge and gingival line. To preserve the alveolar bone, extraction was postponed after disking the central incisors and corticision was done on the ankylosed tooth. The extraction spaces of the central incisors were closed, and the lateral incisors were esthetically restored with resin as a substitute of the central incisors. After five years of retention, stable occlusal relationship, proper overjet and overbite, and good maintenance were confirmed.
